10 Informaciones Psiquiátricas 2021 - n.º 244 ASSESSMENT SCALE OF CONSCIOUS CONTROL OF MOVEMENT IN HUNTINGTON'S DISEASE (CCM-HD). Abstract Introduction: Physical therapy is recommen- ded for the treatment of Huntington's disease. The Unitat Polivalent Barcelona Nord of the Hos- pital Mare de Deu de la Merce is a center concer- ted with the CatSalut that offers a comprehen- sive care to patients with Huntington's disease. Objectives: After twelve years of physical therapy with Huntington’s disease patients, we have observed an improvement in aware- ness of voluntary movement that translates into an improvement in the patient's functio- nality. However, when we have tried to objec- tify this improvement, we have had difficul- ties to find scales that are sensitive enough to measure these changes. We propose a nine- item scale that evaluates those elements that patients work in physical therapy. Methods: Thirty participants with Huntington’s disease are included. The feasi- bility, reliability and internal consistency of the scale are evaluated. Likewise, the scale is correlated with other functional, motor, cog- nitive and quality of life assessment scales. Results: The scale is feasible (range of application time: 11.26 - 14.24 minutes) and has internal consistency (Cronbach's alpha 0.99). The reliability of the scale between the same examiner and between two examiners is high for the total score (0.96, 0.97, 0.93 and 0.97) but differs according to the items. Conclusions: The scale is feasible and has internal consistency but requires training to improve intra- and inter-examiner reliability. Introducción La enfermedad de Huntington es una en- fermedad hereditaria, autosómica domi- nante, neurodegenerativa, causada por una expansión inestable de la repetición del tri- nucleótido CAG en el cromosoma 4. Clínica- mente se caracteriza por sintomatología de las esferas motora, cognitiva y conductual. A nivel motor hay sintomatología hipercinéti- ca en forma de movimientos coreicos y dis- tonía, sintomatología hipocinética en forma de hipocinesia y bradicinesia, rigidez, imper- sistencia motora, alteración de la marcha y del equilibrio. En la práctica clínica el trata- miento consiste en el control sintomático. Cada vez hay más interés en el papel que juegan las terapias no farmacológicas como la terapia física 1-3 . La terapia física es recomendable para el tratamiento de la enfermedad de Hunting- ton. La observación clínica, después de doce años de haber iniciado terapia física en pa- cientes con enfermedad de Huntington en la Unitat Polivalent Barcelona Nord, sugiere una mejoría de la consciencia del movimien- to voluntario que se traduce en una mejoría de la funcionalidad del paciente. Sin embar- go, cuando se ha buscado objetivar dicha mejoría, hemos tenido dificultades para en- contrar escalas que sean lo suficientemente sensibles para medir dichos cambios, si bien es cierto que cada vez hay más estudios que investigan la eficacia de la terapia física en la enfermedad de Huntington 4-9 . Hemos elaborado una escala que incluye 9 ítems (ver anexo 1) que se trabajan en las sesiones de terapia física con los pacientes con enfermedad de Huntington atendidos en nuestro dispositivo y a la que hemos llamado “Escala de valoración del control consciente del movimiento en la enfermedad de Hunt- ington” (CCM - HD).
